How To Refer A Patient

To refer a patient, call 909-651-1810

Fax the following paperwork to 909-651-4257:

  • Legible Pediatric Neurology Referral Form signed by the referring physician
  • Medical records including clinical history, laboratory results, neurological imaging, and EEG results
  • Authorization for CPT codes 99241 – 99245 (if required by the patient’s insurance)

Quick Consults

If you need a referral that requires consultation within the week, call 909-835‐1810 and speak to one of our neurologists directly.

If a patient needs a new consult within the week and there are no openings, the referring physician can call the doctor to doctor line at 909-558-0099 for the physician on call.
