CareLink is a secure web portal where providers access patient information from our shared electronic medical record (EMR) system, CareConnect. CareLink also empowers providers with easy, paperless referrals and automatic follow-ups.

Medical Records

Medical records can be obtained at our San Bernardino office from 8 a.m. - 5 p.m. Monday through Friday. Please note there is a $6 fee for obtaining medical records.
